To access the app, members will have to complete an initial consultation with their Tria Health pharmacist and register on our patient portal. The member will then have access to their personalized care plan developed by their Tria Health pharmacist at any time.

If you are currently using a Tria Health blood glucose meter, blood pressure monitor, or one of our other qualified devices, you can now access a mobile dashboard. Youll be able to view a summary of your readings, personalize or add notes, customize your health alerts, and order supplies as needed.
