This App is part of the “Jockey Club Home Health Watch Programme”, which is organized by The Open University of Hong Kong and funded by The Hong Kong Jockey Club Charities Trust, aims to train community volunteers to provide timely, community-based support and care to the elderly, the disabled and individuals suffering from chronic diseases.
