Care Navigator play online

The Nottingham Care Navigator (NCN) is, at present, for General Practitioners and Consultants looking after Nottingham citizens in need of hospital care. The NCN provides access to advice and support from the right clinical service, first time.

The listed services and information have been developed with two purposes. Firstly, to enable GPs to seek Specialist advice directly. GPs can then book a reserved, urgent out-patient appointment to avoid an unnecessary admission if required. Secondly, to enable Consultants in hospital to organise timely follow up in Primary Care.
