New SkyCare play online

The New SkyCare app enables patients with diabetes type 2 or heart failure to track their measurements and symptoms in the scope of the New SkyCare study. The measurements and symptoms are sent to a server and made available to care providers that remotely monitor the patients. The app also informs patients about actions they should perform, e.g. measure blood pressure, and offers educational material on their condition.
