JoinCareTeam - Patient play online

JoinCareTeamproviding you with access to health care from any device, anywhere! You will connect with a care team to create a plan focused on improving your health, your happiness and your livelihood. Do these goals sound like good targets to reach? We certainly think so!

Additional members of the care team may include doctors, nurses, care coaches, peer advocates, social workers, other health care support staff, and even interpreters if needed! Together, you will work your care team to create a plan of action and carefully track progress over time.

You can get started by contacting a local community health center and they may provide you access to JoinCareTeam. From there you will be on your way to improving your health, happiness and livelihood!
